First let me say GROSS! The stains could be anything. He could be ill and have some kind of discharge. But on the other hand, I truly believe in following your gut. If there was a reason for you to check his underwear in the first place there is some kind of problem. The first is trust. Please talk to him and let him know what you seen. After that some counseling wouldn't hurt.
